The pattern name/number is unknown. Collectors call this pattern "Stacked Rings" or "Michelin Man."
The label is one of two different labels that were used on Fry tableware. The one shown is the Beaver label with the words "Fry Glass" on it. The other variation would be a gold colored shield with the initials B.V.Q. (Beaver Valley Quality) with the words Fry Glass flanking the top and bottom of the label.
