The following information is courtesy of Tom Felt, the author of the upcoming book on L. E. Smith Glassware:
"[The horse] was named Carol's Sure Win and was made in crystal and various colors (including some with carnival finishes) for Mary Walrath circa 1979-1981. She was the owner of Cherished Glass Wares (hence the CGW mark). Some sources state that he was made from an older mold acquired from Westmoreland and/or Haley Glass Company, but since the Smith's inventories list a horse vase that was made in the 1940s, I suspect it is Smith's own mold. There is also a 2-1/4" high version of this cutie, which is known to toothpick collectors as "Horse and Leaves." Although I found no absolute evidence for the latter, I assume it is Smith as well."
