Name and pattern number are unknown but the candle holder has been found with a label saying "Glass Art, Owens-Illinois" according to the "Glass Candlestick Book, Volume 3" by Felt and Stoer. The candle holder was designed by William E, Mackelfresh, Jr of Chicago who designed one other holder for this company as well as the Athena holder for Libbey. A patent was approved for this design in 1936.
