Queen Glass Cutting on Cumberland Glass Tall Sherbet
According to member JayGeorge, who has a matching goblet with a Queen Glass Co. label, the blank would have been made by The Cumberland Glass Company (1932-61).
The blank was decorated by The Queen Glass Company (1929-1987). A key feature of their version of this cutting are the polished dots above the prism (as opposed to gray cut on other productions.)

The pattern name is unknown.

This stem is shown as "UNK 267" on replacements.com and is described as "cut floral, cut stem."
